Pathways to Collaboration:

The Community Learning Series provides accessible, evidence based education designed to strengthen dementia informed communities. Offered as single sessions, multi session series, workshops, or seminars, these programs engage the broader Central Iowa community—not just clients at Perennial Pathways—and aim to equip individuals with dementia, families, care partners, and other community members with practical knowledge and skills they can apply in everyday life.

The content is fresh, engaging, and actionable, offering concrete strategies that help community members build resilience and confidence. Because dementia touches all of us—at some point and with increasing frequency—these sessions ensure that the wider community is prepared with tools that make a meaningful difference.

The Professional Learning Series offers advanced, evidence based training designed to strengthen dementia informed practice across disciplines. Delivered through workshops, seminars, and CEU eligible programs, these sessions provide mental health professionals, medical providers, and allied health practitioners with the latest research, practical strategies, and clinical tools to integrate dementia informed approaches into everyday care.

By focusing on skill building, resilience, and collaborative care, the series equips professionals to better support individuals with dementia and their care partners—while fostering a network of providers committed to excellence in dementia informed practice.

Organizational Consultation & Clinical Service Agreements

Perennial Pathways partners with senior living communities and other care organizations to deliver dementia informed services that enhance both staff expertise and resident well being. From the Heart at Home programming for residents in independent or assisted living to staff dementia behavioral coaching and ongoing staff education plans, our tailored agreements provide sustainable, evidence based support designed to meet the unique needs of each organization.
