Who We Serve

At Perennial Pathways, we provide care for individuals with mild cognitive impairment (MCI) or dementia—whether newly suspected or already diagnosed. Our evaluations go beyond diagnosis to highlight cognitive strengths, areas of challenge, daily functioning, and decision making abilities.

We believe a diagnosis is only one part of the story; our focus is on understanding the whole person—their history, values, and goals—and offering services that empower them to live meaningfully.

We also recognize care partners* as clients in their own right. Caregiving can be deeply rewarding, but it often brings stress, role changes, and vulnerability to burnout. That’s why we provide individual and group services designed specifically for care partners, offering practical tools, emotional support, and a compassionate space to navigate the challenges of dementia caregiving.

Our Unique Model of Care Delivery

Whether you are an individual, family, or care partner seeking clinical services, we are dedicated to opening pathways to our compassionate, evidence-based clinical services through a hybrid model that makes such care more accessible. It complements home and telehealth models by providing a consistent therapeutic environment and access to specialized assessments. To improve access to a high-demand services, scarce resource, Perennial Pathways utilizes an innovative hybrid model that includes:

Clinic appointments are held at our West Des Moines office and are ideal for formal evaluations, psychotherapy, and participation in the R.I.S.E. for Care Partners Program. This structured setting offers a calm, healing space for meaningful engagement outside the home.

Clinic visits can provide respite and privacy for individuals with dementia and their care partners—especially those navigating reduced independence or caregiving fatigue. For many, it’s a chance to step out, reconnect, and do something healing for themselves.

Our Heart at Home Program* brings care directly into the client’s living environment. Designed for those facing barriers to clinic access—such as mobility limitations, transportation challenges, advanced dementia, or limited support—home visits allow for real-time observation of routines and behaviors. This enables highly tailored interventions that reduce distress and improve safety in the home.

Telehealth visits offer a convenient, flexible way for busy care partners or rural Iowans to access care. They reduce transportation challenges, minimize exposure risks, and help maintain continuity during times of transition or heightened stress that can make clinic or home-based care difficult to access.

Telehealth is especially effective for dementia behavioral coaching, feedback, follow-up, or maintenance sessions. These services can also be combined with clinic or home visits as part of Perennial Pathways’ innovative hybrid care model.

Clinical Services

Perennial Pathways offers a comprehensive, holistic intake evaluation tailored to the unique needs, values, and goals of each client with dementia and their care partner. Through a collaborative process, we get to know your story, clarify your goals, and create a treatment plan tailored to your needs. Your welcome packet provides details on how to prepare and what to expect during your first sessions.

Cognitive evaluations may be recommended when further clarity is needed to better understand changes in your or your loved one’s memory or thinking. They may be indicated to clarify diagnosis, identify areas of cognitive strength and challenge, assess functional abilities, or help rule out other potentially treatable causes of cognitive decline.

While using standardized assessments, these evaluations also use a holistic, person-centered approach that goes beyond diagnosis. They provide meaningful insight to guide care planning, support daily functioning and independence, and help you reach your goals with dignity and confidence. This information ensures care is tailored to you and allows us to collaborate in improving your quality of life while supporting care partners and families at each step of the dementia journey.

Psychotherapy or “talk” therapy provides a safe and supportive space to navigate the emotional challenges that often accompany dementia and memory changes—for both individuals and their care partners. Sessions offer guidance in managing depression, anxiety, grief, and the stress that can arise from shifting roles and relationships. Together, we focus on building resilience and confidence, creating a path toward dignity, hope, and meaningful growth even in the face of change. Using approaches like CBT, IPT, Supportive Psychotherapy, and Behavioral Activation, each plan is personalized to fit your unique needs, goals, and values.

Living with dementia or memory loss often brings changes in mood and behavior, such as depression, anxiety, sleep difficulties, irritability, or even paranoia. These experiences can be complex, overwhelming, and deeply distressing for both the individual and their care partners. Support your loved one—and yourself—by learning practical strategies to address the most common and stressful dementiarelated behaviors. Each session offers education, practical tools, and compassionate guidance to help you identify effective solutions and bring relief in the face of dementiarelated challenges. These services are offered both individually and as part of the R.I.S.E. for Care Partners program described below.

Supportive counseling sessions following the loss of a loved one with dementia. Services are limited to care partners already enrolled in Perennial Pathways services.

Care Partner R.I.S.E. Program

Resources, Information, Support, & Empowerment

A structured series of psychoeducational workshops and evidence-based group therapy sessions designed to build skills, resilience, and emotional support. Through education and group connection, care partners gain practical, accessible tools to manage stress and care for BOTH themselves and their loved one with dementia.

At Perennial Pathways, we believe in the power of community and connection to support mental health and wellness. By fostering meaningful relationships with fellow care partners, participants can build a strong network of support that nurtures strength, resilience, and hope.

Perennial Pathways Heart at Home Program

This conciergestyleinhome program delivers dementia care directly where challenges most often arise. It combines functional and behavioral assessments, care partner coaching, adaptive planning, and follow-up treatment. Heart at Home is modeled after evidence-based care models but is the first of its kind in Iowa. It reduces safety risks, empowers care partners, and helps families maintain stability in the home environment.

All out-of-pocket fees will be collected at the time of service.
